Insurance Participation

We participate as an in-network provider for the following plans:

• Delta Dental PPO
• United Concordia’s NFFS plans
• Aetna PPO
• Cigna PPO

We are happy to assist patients with ANY PPO insurance by submitting claims and accepting payment on their behalf. We are NOT able to receive benefits from HMO/DMO insurance plans. Estimated copayments will be collected at time of service for all plans, as dictated by the plan. Any residual copayment due after insurance payment will be billed promptly for payment by the insured party.

Although we do our very best to be informed about insurance coverage, please be aware that there are literally thousands of plans and it is impossible to know the details of every employer’s individual plan details. Your insurance is a contract between you, your employer, and the insurance company. As an outside party, our ability to secure details can be limited. Please participate in your coverage so that we may work together to help you receive your best benefit.
