We would like to make your first visit with us as stress –free as possible. You can assist us by completing your new patient registration forms prior to your appointment and at your convenience. Please print and complete one of each form that is applicable.

Medical History Form

For reference, we will need bitewing x-rays taken within 1 year and a full-mouth or panorex x-ray taken within 5 years (2 years for periodontal patients). These radiographs make a complete record. If you are not able to retrieve a copy, we will need to take one for your record.
